光照时长对老年2型糖尿病患者维生素D和血脂的影响

摘要:

目的 探讨不同光照时长对老年2型糖尿病(T2DM)患者维生素D、血脂水平的影响。方法 选取2022年5月—2023年5月于兰州大学第一医院住院治疗的395例老年T2DM患者(男性206例,女性189例),年龄中位数为70.20(67.30, 74.40)岁。收集研究对象一般资料、接受光照时长、血脂谱、25-羟维生素D[25(OH)D]质量浓度等指标。根据光照时长四分位数将患者分为Q1、Q2、Q3、Q4组,比较各组患者血脂、25(OH)D质量浓度等差异。根据不同25(OH)D质量浓度将患者分为维生素D缺乏及维生素D不缺乏组,比较2组患者生化指标差异。Spearman相关性分析光照时长与血脂、25(OH)D质量浓度相关性。二元Logistic回归分析光照时长与维生素D缺乏关系。结果 Q4组25(OH)D质量浓度较Q1、Q2组升高(P<0.05),总胆固醇(TC)低于Q1组(P<0.05)。随着25(OH)D质量浓度升高,TC、甘油三酯(TG)水平降低。光照时长与25(OH)D质量浓度呈正相关关系(rs=0.365,P<0.001),与TC(rs=-0.244,P<0.001)、TG(rs=-0.229,P<0.001)呈负相关关系。二元Logistic回归显示光照时间较长组维生素D缺乏发生风险降低。结论 适当延长光照时长可改善老年T2DM患者TC、TG水平及25(OH)D质量浓度。维生素D缺乏者TC、TG水平升高。延长光照时长与维生素D缺乏发生风险降低相关。

Abstract:

Objective To investigate the effects of different light durations on vitamin D and blood lipids in elderly patients with type 2 diabetes mellitus(T2 DM). Methods 395 elderly patients(206 males and 189 females) were selected, their median age being 70.20(67.30, 74.40) years old were randomly selected from May 2022 to May 2023 in The First Hospital of Lanzhou University. Their general data, duration of exposure to light, blood lipid profile and 25-hydroxy vitamin D [25(OH)D] level were collected. According to the quartile of light duration, the patients were divided into Q1, Q2, Q3 and Q4 groups, and the levels of blood lipids and 25(OH)D in each group were compared. According to the different levels of 25(OH)D, they were divided intoa vitamin D deficiency group and a non-vitamin D deficiency group, and the differences of biochemical indexes in each group were compared. Spearman correlation analysis was used to analyze the correlation between light duration and blood lipids and 25(OH)D levels. Binary Logistic regression was used to analyze the relationship between light duration and vitamin D deficiency. Results The level of 25(OH)D in Q4 group was higher than that in Q1 group and Q2 group(P<0.05), total cholesterol(TC) was lower than that of Q1 group(P<0.05). With the increase of 25(OH)D, the levels of TC and triglyceride(TG) decreased. The duration of illumination was positively correlated with the level of 25(OH)D(rs=0.365, P<0.001), and negatively correlated with TC(rs=-0.244, P<0.001) and TG(rs=-0.229, P<0.001). Binary Logistic regression showed that the risk of vitamin D deficiency decreased in the group with longer exposure time. Conclusion Light can improve the levels of TC, TG and 25(OH)D in elderly patients with T2DM. The levels of TC and TG are increased in patients with vitamin D deficiency. Prolonged exposure to light is associated with a reduced risk of vitamin D deficiency.
