目的 探讨体重调整腰围指数与女性2型糖尿病合并尿失禁之间的关系。方法 随机选取2022年5月—2023年7月于兰州大学第一医院住院治疗的374例2型糖尿病(T2DM)女性患者为研究对象,收集其临床资料。按是否合并尿失禁,分为T2DM组和T2DM合并尿失禁组,比较各组生化指标。根据体重调整腰围指数三分位法将所有患者分为3组,比较各组尿失禁及其亚型的患病率。采用Logistic回归分析女性T2DM合并尿失禁发生的影响因素。分层Logistic回归分析体重调整腰围指数与女性T2DM合并尿失禁及亚型的相关性。采用受试者操作特征曲线评价体重调整腰围指数对女性T2DM合并尿失禁的预测价值。结果 基线资料分析发现,T2DM合并尿失禁组体重调整腰围指数高于T2DM组的(P<0.05);在高水平体重调整腰围指数组中,尿失禁100例,发生率为80.0%、压力性尿失禁48例,发生率为38.4%、急迫性尿失禁39例,发生率为31.2%;在低水平体重调整腰围指数组中,尿失禁38例,发生率为30.4%、压力性尿失禁和急迫性尿失禁均16例,发生率均为12.8%;回归分析显示,高水平体重调整腰围指数与尿失禁、压力性和急迫性尿失禁相关,Logistic回归分析进一步发现体重调整腰围指数是女性T2DM合并尿失禁发生的危险因素。结论 女性T2DM人群中高水平体重调整腰围指数患尿失禁、压力性尿失禁和急迫性尿失禁的风险增加,应积极干预和防治。
Abstract:Objective To explore the relationship between weight-adjusted waist index and urinary incontinence in women with type 2 diabetes mellitus. Methods 374 female patients with type 2 diabetes mellitus(T2 DM) who were hospitalized in The First Hospital of Lanzhou University from May 2022 to July 2023 were randomly enrolled in the study, and their clinical data were collected. They were divided into T2 DM group and T2 DM combined incontinence group according to whether they were combined incontinence or not, and the biochemical indexes of each group were compared. The prevalence of urinary incontinence and its subtypes in each group was compared by weight-adjusted waist index tertile method. Logistic regression was used to analyze the factors affecting the development of urinary incontinence in women with T2 DM. Stratified Logistic regression was used to analyze the correlation between weight-adjusted waist index and the combined urinary incontinence and subtypes of T2 DM in women. The predictive value of weight-adjusted waist index for urinary incontinence in women with T2 DM was evaluated using the subject operating characteristic curve. Results Analysis of the baseline data showed that the weight-adjusted waist index of the T2 DM combined incontinence group was higher than that of the T2 DM group(P < 0.05). In the high-level weight-adjusted waist index group, there were 100 cases of urinary incontinence, with an incidence of 80.0%, 48 cases of stress incontinence, with an incidence of 38.4%, and 39 cases of urge incontinence, with an incidence of 31.2%. In the low-weight-adjusted waist circumference index group, there were 38 cases of urinary incontinence, with an incidence of 30.4%, and 16 cases of stress incontinence and urge incontinence, both with an incidence of 12.8%. Regression analysis showed that high levels of weight-adjusted waist index were associated with urinary incontinence, stress and urge incontinence. Logistic regression analysis further revealed that weight-adjusted waist index was a risk factor for the development of combined urinary incontinence in women with T2 DM. Conclusion High levels of weight-adjusted waist circumference index in the female T2 DM population are at increased risk for urinary incontinence, stress incontinence, and urge incontinence and should be aggressively intervened and prevented.
