视觉线索训练对帕金森病伴冻结步态患者抑制性控制功能的影响

摘要:

目的 探讨视觉线索康复训练对帕金森病(PD)伴冻结步态(FOG)患者大脑抑制性控制功能的影响,并分析其与运动状态改善之间的关联性。方法 选取72名PD伴FOG患者,通过简单随机法分为常规药物治疗组(对照组)与常规药物治疗联合视觉线索训练组(试验组)。采集治疗前及干预一周后的抑制性控制任务(Go/NoGo)功能评分,FOG量表(FOG-Q)及简易平衡评定系统测试(mini-BESTest)评分。干预前组间比较采用独立样本t检验;干预后组间比较以基线值为协变量采用协方差分析。采用Pearson相关分析评估大脑控制能力与运动功能的相关性。结果 两组在性别、年龄、受教育程度及病程的差异均无统计学意义(P > 0.05)。经视觉线索训练,试验组NoGo错误率显著降低(F=4.471,P=0.038),FOG-Q评分显著降低(F=31.477,P<0.001),mini-BESTest评分显著提高(F=40.163,P<0.001)。试验组FOG-Q与NoGo错误率呈正相关关系(r=0.480,P=0.005)。结论 对PD伴FOG患者进行视觉线索训练有助于患者恢复大脑抑制性控制功能,改善FOG症状与平衡功能。

Abstract:

Objective To explore the effect of visual cues rehabilitation on the control function of freezing of gait(FOG) in Parkinson's disease(PD) and analyze the correlation between control abilities and motor status. Methods Seventy-two patients with PD and FOG symptoms were recruited and divided into a conventional drug treatment group(control group) and a visual cues rehabilitation care treatment group(experimental group) by a simple randomization method. Functional scores of the inhibitory control task(Go/NoGo), FOG questionnaire(FOG-Q) scale, and the mini-balance evaluation systems test(mini-BESTest) were collected before treatment and one week after intervention. Before the intervention, the two-sample t-test was employed to compare the two groups, and after that, an analysis of variance was employed to compare the groups while baseline values were controlled as covariates. The Pearson correlation was used to analyze the correlation between cognitive control and motor function. Results There were no significant differences in gender, age, education level and duration of disease between the two groups(P > 0.05). After visual cues rehabilitation nursing, NoGo error rate was significantly decreased(F = 4.471, P = 0.038), FOG-Q score significantly decreased(F = 31.477, P < 0.001), and mini-BESTest score significantly increased(F = 40.163, P < 0.001) in the experimental group. The freezing gait symptom scores were positively correlated with NoGo error rate of the inhibitory control function(r = 0.480, P = 0.005) in the experimental group. Conclusion Visual cues rehabilitation for PD with FOG patients may help patients restore their inhibitory control function, improve frozen gait symptoms and enhance balance functions.
